How To Fill Out The Portable Questionnaire
(This
is not the download page. This is just
a showcase of what the questionnaire looks like and how to fill
it out. The questionnaire download happens at the end of the ordering
process.)

Step 1: After
the ordering process, download the Portable Questionnaire. Make
sure you download the Questionnaire that is right for you. We have
the Questionnaires in multiple formats. (.exe, Microsoft Word,
Adobe Reader, and Text) Follow the directions on that page.
 
NOTE:
The Questionnaire is truly PORTABLE. You can take it with
you on any computer you own. It can be filled out at Work or at
Home and there is NO TIME LIMIT on how long you have to fill it
out. You can take as long as you like. Most of our customers
fill it out in under an hour, if they are organized.

Step 2: Save
the Case Questionnaire to your DESKTOP. So you can find it later.

Step 3:
When
the download is complete, check the file for viruses with your virus
scanner. (This is just a good safety precaution for any and all
downloads on the web.)

Step 4:
Open the Questionnaire and begin to fill it out. This
is pretty easy if you are organized and have your information all
in one place. It usually takes under an hour.

Step 5: When finished, submit your
Questionnaire back to us by email or fax. Make sure that we know
that your questionnaire goes with the name you paid under.
